Humanity in Transition Inc. (HIT), is a minority owned 501c3 organization that was established in December of 2017 by Owner and Founder, Territa Wilkinson. HIT has evolved into a wrap-around organization who provides services to a wide variety of individuals, that includes persons with intellectual/developmental disabilities, mental illness, as well as older adults and youth.

Our mission is to empower and engage the people we service while enhancing our participants quality of life. To utilize a holistic approach to care that moves beyond just the physical, striving to meet the needs of the whole person, mind, body, and spirit, to provide a safe and secure environment that fosters independence and transforms lives, allowing every person to flourish in transition.

People Driven

We are a people driven organization that is working to assist vulnerable populations in the community. We believe that everyone is in transition and that we can all benefit from assistance that is not a one size fit all approach. Our caring, licensed, professional, staff provides personal attention and tailors person centered care to meet the needs of each person.

Senior Day Program
​
We currently have an Adult Supportive Day Program located in Washington, DC 20019 serving Ward 7. This program is designed to meet the needs of seniors and adults with disabilities, who live in the community. This program is currently privately funded and free for all participants.
We offer socialization, assistance with personal care and a wide variety of activities to help the individuals who attend our adult day program, maintain their independence and feel empowered to maintain in the community in which they live.
Program Eligibility:

Any resident of the District of Columbia who is 55 years of age or older, living anywhere in the District may enroll in the program.
Acceptance Criteria:
Participants must meet the following criteria for acceptance to Humanity in Transition Senior Day Program:

Services
Companion Care
Companion services provide non-medical assistance and supervision to support a person’s goals, desires, and needs as identified in the person’s Individual Support Plan (ISP) and reflected in his or her Person-Centered Thinking and Discovery tools. Person’s supported goals may be related to the person’s safety, promotion of independence, community integration, and/or retirement.
Individualized Day Services
IDS is a tailored day service designed to maximize a person’s individualized choice and needs either in a 2:1 or 1:1 ratio. These tailored activities provide unique opportunities for increased integration and inclusion as well as further opportunities for the person to form valuable relationships not to mention possibly obtaining employment and volunteer experiences.
Adult Day Supports
Our facility will facilitate an array of medical, psychosocial, community-based, and person centered services. We realize that as more people decide to age in the homes opposed to institutions which are not cost effective or person centered, that it is paramount that we develop a cadre of services specific to the needs of the participants seeking ADHP services from HIT.
